Guarantee Terms and Conditions
This guarantee is subject to the following conditions: This guarantee is based on the cost of your membership and does not include your enrollment fee. MaternityCard members must complete their 12 month contract and have met all payment terms for their plan. All monthly payments must be made on time. MaternityCard members must REGISTER this Certificate of Guarantee, after enrolling, by mailing or faxing back to us the signed HIPAA Compliance Form within 30 days of membership. MaternityCard members must submit all maternity claim forms to the Patient Advocacy Team prior to review request. MaternityCard members must only use our program to reduce their bills use of insurance, Medicaid, charity, or any other arrangements with any provider voids the guarantee. MaternityCard members must deliver at a hospital facility in the U.S. (If using a birthing center or having home birth, the guarantee does not apply.) Members agree to submit all medical bills regarding maternity to AHCO Patient Advocacy Team. Failure to present these bills and forward specified payments to the provider within the specified time will void guarantee. Members must submit copies of receipts with this request to verify timely payments of bills. If receipts are not received within 30 days of original cancellation request, review will be denied. The $200 bonus of this guarantee does not apply to our negotiated service plan. Guarantee is not effective for our Pre-Pregnancy Plan until member is confirmed pregnant and has met all other requirements above. Failure to respond to AHCO inquiries in relation to medical bills will void the guarantee. AHCO reviews Memberships Request on a case by case basis. The review may take up to 90 days. The Membership Review Request form must have been received within 30 days of receiving your cancellation request. The review request will not be processed unless AHCO has received your cancellation request form.
